Virtual appointments are provided by phone or via secure video link through the PharmaConnect platform.
PROGRAM DETAILS
The goal of this program is to transition patients safely and seamlessly from hospital to home sooner by providing Pharmacist led virtual diabetes education and monitoring at home. It is a 14-day program intended for transition from Hospital à Discovery Pharmacy à Home Pharmacy.
The intended outcomes of the Diabetes Integrated Care (IC) Pathway are to:
Deliver comprehensive, virtual diabetes education in the home to enhance quality of care and patient experience.
Safety monitor patients with diabetes at home to avoid preventable re-hospitalization.
During the 14-day period, Pharmacists at Discovery Pharmacy provide one-on-one education to the patient on:
Overview of diabetes
Blood glucose monitoring
Nondrug therapy (lifestyle modifications – diet, exercise)
Insulin start/dose titration
Additional support and diabetes resources
Contact Points
The patient’s home pharmacy will be notified via fax by Discovery Pharmacy once enrolled in the transitional diabetes education program.
Initial Visit appointment will be entered into EPIC by the UHN Connected Care team as a phone appointment.
Follow-Up Visits will be entered into EPIC by Discovery Pharmacy as phone appointments.
If escalation is required, Discovery Pharmacy will communicate the reason for escalation and recommendation(s) to the UHN Connected Care team via eFax.
Program end is at 14 days or when patient has been assessed as ready for transition back to community. Communication regarding this transition will be sent to UHN Connected Care team, the patient’s primary care provider, and the patient’s home pharmacy.
APPOINTMENT TYPES
There will be four appointments scheduled during the 14-day period.
Day 1-3:Initial Visit
Day 4-7: Follow-up and Education Session
Day 8-11: Follow-up and Education Session
Day 12-14: Follow-up, Education Session, Final Assessment
