Follow-up consultations

Our pharmacy team would like to schedule a time to follow up with you regarding what was discussed at your most recent appointment. 

The purpose of this appointment is to ensure that your treatment plan is progressing as expected and to address any additional questions or concerns that may have come up after beginning medication therapy or implementing recommendations.  

The consultation generally takes 5-7 minutes and is scheduled for 20 minutes to provide additional time for discussion, if necessary.  

After the consultation, any outstanding issues will be addressed and followed up on by the pharmacy team.  

Virtual Consultations

Virtual Consultations are provided by phone or via secure video link through the PharmaConnect platform. In-Person appointments are also available.
